Home, Hope and Healing (HHH) has a full-time job opening for a Personal Care Field Coordinator. The position works at our central office in Smithfield and travels throughout our Personal Care Agency (PCA) coverage area. Works weekdays, 8:00 a.m. – 4:30 p.m. with occasional flexibility after-hours based on client and program needs. 

Responsibilities include:
The PCA Field Coordinator facilitates the quality and growth of our personal care program by efficiently completing supervisory visits to our PSS (Personal Support Specialist) field staff, admissions visits and “meet and greet” visits. Performs company orientation for new PSS staff and follows up with new hires to ensure smooth onboarding/acclimation to the PSS role. Conducts observed performance visits to new PSSs at 30-, 60-, and 90-day intervals, 30-day follow-up conversations, and coaching at client visits as needed. Ensures positive customer service and accurate, thorough, and timely communication in all interactions with clients/families and HHH professional and paraprofessional staff. Reports to the PCA Program Manager. 
 
Job Requirements:
  • PSS certification with at least one year of experience as a PSS, or registered as a CNA, or has a combination of at least 5 years of education and/or experience in a related field. Familiarity with the home care field is a plus. AS degree with 2+ years of office experience is preferred
  • Good communication and listening skills with the ability to professionally answer questions and manage conflict from clients, families, staff, and outside organizations
  • Ability to prioritize work and meet deadlines
  • Reliable and strong work ethic, with excellent customer service skills
  • Respect for client/patient confidentiality and privacy
  • Good computer skills: ability to enter data and log information into computerized databases; ability to use MS Word, MS Excel, and MS Outlook
  • Adult Mandated Reporter training
  • Reliable transportation and good driving record
